Nightingale Nursing Services Inc. (“Nightingale”, “we”, “us”) values your privacy. This notice explains what personal and sensitive personal information (“Personal Data”) we collect, why we collect it, how we protect it, and the rights you have under the Data Privacy Act of 2012 (Republic Act No. 10173).
Personal Data we collect
From clients and their families
- Name, phone number, email address and location
- Details about the patient needed to arrange care, such as their condition, care requirements and preferred schedule
From applicants and Healthcare Associates
- Name, contact details, profession and preferred location
- Resume, photograph, employment history and references
- Academic records, professional licenses, certifications and clearances
- Government-issued identifiers and documents submitted as application requirements
- Results of interviews, assessments, physical examinations and background checks
From visitors to this website
- Information you enter in our request and application forms. Submitting a form opens your own email application, and the details reach us only when you send that email.
- A record, stored in your browser only, that you have accepted this Privacy Notice.
How we use Personal Data
- To respond to inquiries and requests for care
- To recommend and deploy an appropriate nurse, midwife or caregiver
- To screen, verify and assess applicants and maintain our registry of Healthcare Associates
- To coordinate with clients, associates and healthcare institutions during an engagement
- To comply with legal, regulatory and accreditation requirements
Who we share it with
We share Personal Data only as needed for the purposes above: with the Healthcare Associate assigned to a case, with the client or healthcare institution an associate is deployed to, with service providers who help us operate, and with government authorities when the law requires it. We do not sell Personal Data.
How long we keep it
We keep Personal Data only for as long as needed to fulfil the purposes for which it was collected, or as required by law, after which it is securely disposed of.
How we protect it
We apply reasonable organizational, physical and technical measures to keep Personal Data secure and confidential, and we limit access to personnel who need it to do their work.
Your rights
Under the Data Privacy Act of 2012, you have the right to be informed, to access, to object, to correct, to erasure or blocking, to data portability, to damages, and to lodge a complaint with the National Privacy Commission.
